Survey about Alternative Public Library Services

I am a currently conducting a survey of non-branch alternative library services in North America (examples of these services could include: a bookmobile or van, pop-up library or depot, homebound delivery, kiosk or vending machine, interlibrary loans, or talking books). The survey consists of 28 questions and should take no more than 30 minutes to complete.
